The Patient Care & Wellness Advocate(PCWA) will play a crucial role in enhancing our patient's experience through the following responsibilities:

  • Collaborate closely with physical therapy clinicians to craft follow-up activities and/or referrals for patients, leveraging generative artificial intelligence to enhance patient care and support. This collaboration is vital for patients facing emotional challenges with their therapy.
  • Conduct follow-ups with patients, particularly those facing emotional challenges, to support their therapy journey and boost motivation.
  • Manage patient scheduling
  • Act as a liaison with behavioral health networks, facilitating referrals to ensure comprehensive patient care.
  • Schedule appointments efficiently, ensuring a smooth and organized calendar for our patients and therapists.
  • Collect payments post-therapy sessions, managing financial transactions with accuracy and confidentiality.
  • Answer phones, act as a patient liaison, answer any questions from potential or current patients
  • Perform wellness check-ins, adopting a compassionate approach to assess patients' well-being and satisfaction with their therapy progress.

  • Gather new patient data; keep track of all patient referrals
  • Disseminate information to patients; act as a go-between for patients and physicians

Qualifications:

  • AS or BS in Psychology, Sociology, or commensurate experience. Relevant experience in customer service, healthcare, or counseling is highly valued.
  • Exceptional interpersonal and communication skills, with the ability to empathize and connect with patients.
  • Strong organizational skills, with the capacity to manage multiple tasks and appointments simultaneously.
  • Proficiency in handling financial transactions and maintaining confidentiality.
  • Bilingual abilities are a significant plus, enabling us to serve a diverse patient population more effectively.
