JOB TITLE: Physical Therapist
REPORTS TO: Clinic Founder and Owner
HOURS: Full-Time Exempt Position
CLINIC OVERVIEW: FYZICAL Therapy & Balance Centers – San Francisco was born out of West Portal Physical Therapy & Wellness Center (WPPT), which was founded in 1992 with the goal of providing patients with results-oriented physical therapy in a professional and caring environment.
In 2019, as part of a continuous effort to expand services to our patients, WPPT moved under the national umbrella of FYZICAL Therapy & Balance Centers and relocated to a newly designed clinic at the base of Twin Peaks. FYZICAL, now with more than 450 affiliated clinics nationwide, provides our clinic with extensive resources in multiple areas including clinical and business development. Our facility offers four, private treatment rooms and an airy gym with 12-foot ceilings.
As a team, we are 100% focused on achieving the health and wellness goals of our patients. The clinic provides a full range of orthopedic physical therapy and sports medicine services; however, it focuses on spine care, shoulder and knee rehabilitation, balance, and fall risk screening. We also offer a semi-private, by-appointment Fitness Training Program under the direction of personal trainers.
COMPENSATION: Exempt position. $90,000 - $105,000 depending upon experience. Annual salary review.
$5,000 annual, performance-based bonus paid quarterly. $3,000 signing bonus paid 50% upon commencement of work and 50% upon commencement of year two. Relocation assistance can be discussed.BENEFITS: Following 90-day introductory period, employees in this role are eligible for holidays (11 upon hire), personal days (3), vacation (one week after year one, two weeks after year two, and three weeks after year three), sick leave, bereavement leave, and 50% paid health, dental, and vision insurance. Potential for partial or full clinic ownership over time.

typically has a steady volume of patients, and the noise level in the work environment is usually moderate.
CLINIC HOURS: 8:30 a.m. – 5:30 p.m., Monday to Friday
Responsibilities
JOB DESCRIPTION: Essential functions include, but are not limited to:
- Assess and evaluate physical therapy needs for specifically assigned patient populations with the goal of billing 4.6 units per hour.
- Create, implement, monitor, and modify treatment plans and protocols to achieve established treatment goals and outcomes according to current professional and health system standards
- Implement full treatment program; however, may delegate appropriate portions of patient plan to support personnel
- Develop home exercise programs with patient, family, and caregivers as appropriate and facilitate patient education and self-care recommendations
- Assign and supervise activities of PT assistant and PT aides
- Insure full documentation of treatment plan, patient activity, and progress via electronic medical records (EMR) system
- Prepare initial evaluation with treatment plan and patient progress reports for referring physicians and act as case-specific consultant to physicians and other health care professionals as necessary
- Manage patient discharge planning and prepare discharge summaries to allow for maximum benefits from physical therapy within insurance guidelines
- Identify and refer PT patients to in-house, semi-private exercise program as appropriate
Skills & Qualifications
DESIRED SKILLS AND EXPERTISE: - D.P.T. degree in physical therapy
- License to practice physical therapy in the State of California
- National Provider Identifier (NPI) preferred or willingness to obtain NPI/Medicare certification
- CPR certification
- Minimum of one-year experience as a physical therapist in an out-patient physical therapy clinic preferred. New graduates will be considered.
- Strong interpersonal skills and ability to work well with limited supervision
- Ability to read, write, speak, and follow verbal and written instruction in English
- Basic computer skills required for electronic entry of patient and contact tracking data. Understanding of customer relationship management (CMR), and database, internet, spreadsheet, and word processing software. Ability to analyze and manage multiple concrete variables in standardized software.
